CLTS Community Integration Services
We offer intensive case coordination by licensed social workers to assist, empower, and build upon the strengths of the participant and their family so the participant can be fully integrated into their community. We focus on developing a creative and flexible continuum of care. With an array of experience across service needs, our team will provide fresh insight to meet your needs.
IRIS Support Broker Services
We offer personalized support services to assist in managing your long-term care needs. Some of the services we offer are service coordination, person-centered planning, advocacy, empowerment, authorized representative services, recruiting and managing workers, medical liaison, and more.
